Find your NHS Number

Your NHS number is a 10 digit number, like 485 777 3456.

You do not need to know your NHS number to use NHS services, but it can be useful to have it and will help us with your registration. You can also get your NHS number in your NHS account.

Complete online registration form

Our online registration form collects your details, a brief outline of your medical history and records your preferences. This information is added to your record and will allow us to begin to provide you with medical services.

Please complete the online form to ensure you can be fully registered with us. Please allow 7 days before your registration will be complete and speak to the practice to confirm your registration. If you prefer to register in person please call the reception team.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).

Catchment area

We can only register patients who currently live within our catchment area. Please enter your postcode below to check if your address is within our area.

We only accept those living in Sandwell and includes:

  • West Bromwich
  • Oldbury
  • Tipton
  • Wednesbury
  • Rowley Regis
  • Smethwick

Change of Details

Please ensure you notify us of any change of address/telephone number or personal circumstance. Please bring in your proof of new address.

It is important you inform us as soon as possible as it will help us to keep records up to date should we need to contact you for any reason or refer you to other services.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
